The following forms are currently on sale:

  1. Delta State Colleges of Nursing Sciences admission form for the 2023/2024 session:
  • General Nursing (3 years) at Agbor, Warri, and Eku.
  • Basic Midwifery (3 years) at Asaba and Sapele.

Admission Requirements:

Applicants must possess a minimum of five (5) credits at SSCE, obtained in not more than two (2) sittings. The credits must include English Language, Mathematics, Biology, Chemistry, and Physics.

Interested individuals can obtain the forms at their preferred colleges: Agbor, Warri, and Eku, as well as at the Department of Midwifery in Asaba and Sapele. The cost of the form is non-refundable and amounts to N15,000 (Fifteen Thousand Naira only).
